The Position

The Empowerment Advocate will be a member of the YouthBuild Construction Plus Team. The will be primarily responsible for providing comprehensive human and social services support, and case management.

Job Role and Responsibilities

  • Conduct assessments
  • Provide client centered case assignments, draft service plans, review and track case progress and determine case closure
  • Coordinate and enure that the care that is provided is safe, timely, effective, efficient, equitable and assessible.
  • Help clients move for self sufficency.
  • Facilitate multiple care aspects (case coordination, information sharing, etc)
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Develop effective working relations and cooperate with medical team throughout the entire case management process
  • Navigage needed services through the greater social services ecosystem (Unite US Georgia)
  • Go the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
  • Record cases information, complete accurately all necessary forms and produce statistical reports
  • Promote quality and cost-effective interventions and outcomes
  • Assess and address motivational and psychosocial issues
  • Adhere to professional standards as outlined by protocols, rules and regulations

Requirements and skills

  • Proven working experience in case management, including as a social worker, human services provider, or related job.
  • Excellent knowledge of case management principles, healthcare managementand reimbursement
  • Previous experience with psychological aspects of care
  • Effective communication skills
  • Excellent organisational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills
  • Current license, certification or registration
  • BS degree in related healthcare field

Outcome tracking—track the stories and successes of program participants.

  • Maintain contact with the client and monitor the status of their applications, etc.
  • Use Efforts to Outcome (ETO), a client management system, to document and reflect the outcome of their client activities and progress; maintain data accurately and timely.

Must Have

  • Previous experience in the following roles: social services program case management, income support specialist and community liaison.
  • Knowledge of social benefits programs (counseling, rules, and requirements)
  • Previous work experience with special populations; the ability to provide services in a culturally sensitive manner
  • Able to build and maintain relations internally and externally
  • Communicate effectively with clients and team members.
  • Strong data management skills and report writing
  • Must complete and maintain files for each client online and paper
  • Proficient in Microsoft Word, Excel, and PowerPoint
  • Able to work successfully in a program/project management structure
  • Strong presentation and facilitation skills; also able to develop professional PowerPoint presentation
  • Internet and resource savvy

Education, and Experience

  • Bachelor’s Degree from a four-year accredited institution required in Human Services, Social Work or Clinical Social Work. LCSW a plus.
  • 3+ years of experience in all areas of responsibility
  • Job Type: Full-time
  • Pay: $55,000.00 – $60,000.00 per year
